You may mail or fax the form to the address below, or you can email Dr. Blanco to reserve a spot at the training workshop and bring it with you.
Sorry, an online application form is not available since the form requests a signature from you, and from your school's administration.
Existing Project ASTRO partners need not fill out a new form.

The deadline for applications is October 12, 2012. (Later applications will be considered on a space-available basis. However, please keep in mind that astronomer volunteers are in short supply).
